The report is due October 15, 2009

To complete the form you will need to download the file to your desktop/hard drive.  Title your file specific to your site.  The Annual Report is a Microsoft Word 2007 file.  Hard copies of the Annual Report Form are no longer available.   Alaska’s Annual Report Forms are unique to Alaska Schools and are the only reports that will be accepted by the Alaska Committee in 2009.

Schools must select the appropriate school category from those listed (These are large documents and may take awhile to up-load):

• Annual Report Scoring Guide

• High School

• Middle School

• Elementary School

• K-12 School

• Special Purpose School

• Distance Education School

• Post Secondary School

• Supplemental

 After downloading the file to your desktop/hard drive complete the Annual Report form, e-mail a copy to stowell@alaska.com and fax a copy of the signed demographics page to 907-349-2636.

All Alaska accredited school's Annual Reports are received and stored electronically.    Schools are responsible to send your report to your Superintendent and then to Stowell via E-mail.  Thanks for your cooperation.
